Abstract:

يتميز مرض الكلى الناتج عن السكري (DKD) بارتفاع إفراز الألبومين (الزلال) في البول أو انخفاض معدل الترشيح الكبيبي للكلى أو كليهما. ويعتمد التشخيص السريري لمرض الكلى الناتج عن السكري ال (DKD) لدى الأشخاص المصابين بداء السكري من النوع الثاني على استخدام اختبارات الزلالي في البول والكرياتينين في الدم. ولاستكمال التقييم السريري ل(DKD) ضرورة أستخدام مؤشرات حيوية جديدة تساعد على تحسين تشخيص المرض ومراقبته. كان الهدف من هذه الدراسة هو قياس ارتباط مستويات مادتي الكرياتينين والسيساتين سي وتعدد أشكال الجينات (ACE1 (I/D) مع تطور مرض الكلى السكري بين المرضى السودانيين المصابين بداء السكري من النوع الثاني ومقارنة معدل الترشيح الكبيبي (eGFR) لمرضى السكري المقدر باستخدام معادلات GFR المختلفة. أجريت هذه الدراسة في مركز مرضى السكري بمستشفى كوستي التعليمي بولاية النيل الأبيض. شملت الدراسة 250 حالة مصابة بالسكري و150 حالة صحية. تم جمع عينات الدم بعد موافقة مستنيرة. تم إجراء قياسات نسبة الجلوكوز في الدم أثناء الصيام ومستوى الكرياتينين في الدم والسيساتاتينC باستخدام محلل كيميائي شبه آلي Mindray BA-88A وProtein Analyzer-PA50-Genrui. تم إجراء معدل الترشيح الكبيبي المقدر (eGFR) ومراحل DKD استنادًا إلى أربع معادلات مختلفة وهي eGFR: MDRD وCKD-EPI Creatinine وCKD-EPI Cystatin C وCKD-EPI Creatinine/Cystatin. تم إجراء عزل الحمض النووي باستخدام طريقة التمليح لكل عينة. تم تقييم تعدد الأشكال ACE1 (I/D) للجينات باستخدام PCR. تم جمع عينات البول من المرضى واختبارها للكشف عن الالبيومين . وتم تحليل البيانات باستخدام برنامج SPSS (الإصدار 20؛ SPSS Inc.، Chicago، IL) وتحليل الانحدار الخطي. اعتمادا على ألبومين البول الإيجابي كعلامة تشخيصية لـ DKD، أظهرت دراستنا معدل حدوث ال DKDبنسبة (38٪) بين مرض السكري من النوع الثاني. وكان هناك فرق ذو دلالة إحصائية في العمرp.value= 0.02))، وجلوكوز الدم الصائم ومدة مرض السكري(p.value= 0.001) بين المجموعتين الإيجابية والسلبية لـ DKD. أيضا هناك فروق ذات دلالة إحصائية في مستويات الكرياتينين وسيستاتينسي p.value= 0.001)) بين المجموعتين. وكشف التنميط الجيني (I/D) ACE1 عن توزيع أعلى بكثير لـ DD متماثل الزيجوت (52.7%) في حالات T2DM مقارنة بالحالات الصحية(الضوابط) (32%) (P.value=0.041) وتم العثور على تردد أعلى ل (D allele) (69%) في حالات T2DM من حالات الضوابط (56%) (P.value 0.051). السيستاتينC هو العلامة الحيوية المتفوقة للكشف المبكر عند مرض الكلى الناتج عن السكري ل(DKD) لدى مرضى السكري من النوع الثاني، كما تعد معادلة CKD-EPI Cystatin C الأداة الأكثر دقة لفحص وتحديد مراحل ال(DKD) ، ويعد تعدد أشكال الجينات (I/D) لـ ACE1 عامل خطر لحدوث الDKD الناتج عن داء السكري من النوع الثاني.

Description:

Diabetic kidney disease (DKD) is characterized by elevated urine albumin excretion or reduced glomerular filtration rate or both. The clinical diagnosis of DKD in people with Type 2 diabetes depends on the use of albuminuria and serum creatinine tests. Supplementing clinical assessment of DKD with newer biomarker has the potential to improve disease diagnosis and monitoring. The aim of this study was to assess the association of serum levels of creatinine, cystatin C and ACE1 (I/D) gene polymorphism with the development of Diabetic Kidney Disease among Sudanese patients with Type 2 diabetes mellitus and to compare the estimated GFR (eGFR) of diabetic patients between different GFR equations. The present study was carried out at the Diabetic Center in Kosti Teaching Hospital, White Nile State, Sudan. The study total 250 diabetic cases and 150 healthy controls. Blood samples were collected after informed consent. Measurements of blood glucose, serum creatinine and cystatin C level were measured using Mindray BA-88A and Protein Analyzer-PA50-Genrui semi-automated chemical analyzer. The estimated glomerular filtration rate (eGFR) and staging of DKD was done based on four different eGFR equations: MDRD, CKD-EPI Creatinine, CKD-EPI Cystatin C, and CKD-EPI Creatinine/Cystatin. The DNA isolation was performed using salting-out method for each sample. ACE1 gene insertion/deletion (I/D) polymorphism was assessed using PCR. Urine specimens were collected from patients and tested for microalbuminuria.The data were analyzed using SPSS (version 20; SPSS Inc., Chicago, IL) software and linear regression analysis. Depending on positive urine albumin as the diagnostic marker for DKD, our study demonstrated prevalence of (38%) DKD among Type 2 Diabetes. There was statistically significant difference in age (p-value = 0.02), fasting blood glucose (p-value = 0.001) and the duration of diabetes (p-value = 0.001) between the DKD positive and DKD negative groups. Also statistically significant differences in serum Creatinine (p-value = 0.001) and CystatinC (p-value = 0.001) levels between the two groups. ACE1 insertion/deletion genotyping revealed a significantly higher distribution of the homozygous DD (52.7%) in T2DM cases than in controls (32%) (P-value = 0.041) and the (D allele) frequency was found to be higher in T2DM cases (69%) than the controls (56%) (P-value= 0.051). Cystatin C is the superior biomarker for early detection of DKD in Type 2 diabetic patients. CKD-EPI Cystatin C equation is the more accurate tool for screening and staging of DKD. ACE1 gene insertion/deletion (I/D) polymorphism is a risk factor for the development of DKD in Type 2 Diabetes Mellitus.
